If you would like to know if Park Avenue Fertility participates with your insurance carrier, please contact the office by email by simply clicking on “Contact Us” or call 203-372-6700. Patients with non-participating insurance carriers are welcome.
Our Patient Care Coordinators in the billing department will contact your insurance carrier to check for any services that are covered by your insurance plan and/or if you are covered under the Connecticut State Mandate. They will work closely with you to maximize each option available to you so that your desired treatment and financial obligation is most beneficial to you. If you do not have any insurance coverage, you may speak with Ann DiCenzo, Sr. Patient Care Coordinator to discuss the option of paying privately.
You may contact our office at anytime and ask to speak to the billing department regarding any financial issues or questions concerning fees and services.
Payment is accepted by cash, check, American Express, Mastercard, Visa or Discover. Financing (payment plans) is available through Capital One Healthcare Finance. Capital One offers fixed interest rate loans starting at 1.99% and no pre-payment penalties. Patients may apply confidentially to Capital One by telephone or electronically on-line for loan consideration. Capital One only contacts the office regarding loan approvals. It does not contact the office regarding loan denials. Upon approval by Capital One, and confirmation by the patient to the office regarding the loan amount to be paid to the office, a loan agreement is requested by the office and forwarded to the patient for review. The obligation process does not begin until the loan agreement is signed by the patient and faxed back to Capital One. Patients may decline the loan agreement at anytime by simply not signing the agreement and the approval offer will naturally expire. An expiration date accompanies loan approvals.
